Esp8266 ESP12 Web Form json

first of all, sorry for my bad english.
i want the follow things to do:
i need a webbrowser form with data input and output boxes. the input files must read from esp8266.
for example:
the input box is like----Name---123456789---nickname-----userid or something else
the data i want write in a variable or and array.
i am looking for json or something.
and of course a thoughts offense
