If not, what would be the closest approximation for embedding on SMT PCB with minimal additions?

Specs:

To be powered by regulated 5V DC, up to 3A,

Output: two independent channels, low on start, outputting 5V up to 600mA

Preferred option: USB socket for ease of programming
