The datasheet specifies that ESP can be used in voltages between 3.0V to 3.6V. Anything else is outside this range and function of the chip is not guaranteed - maybe this is reason for software failures You are facing. Did You check for example power requirements of flash memory inside Your ESP module?

#52333 Ok, so can You please do following things?:
1. flash the module with stock AT firmware and boot it up - this prepare internal wifi configuration area so we can rely that WiFi stack configuration is ok
2. format SPIFFS, for example upload some files using Arduino IDE
3. simplify Your code - do simple steps one after one so we can see where the problem is
TRY TO USE WiFi.persistent(false) SO FLASH WIFI CONFIGURATION WILL NOT BE OVERWRITTEN / at least for now.
I've so far never noticed problem with disappearing files from SPIFFS until I've changed the memory layout in IDE.

The spiffs overwrite is the first time that happened. I have reformatted and uploaded a new spiffs file many times, the key to what is happening here I believe is changing the flash size in IDE
